Every person we support has their own goals, needs and way of life. Our services are planned with each person and their team, and follow their Individual Service Agreement.
Residential Services (Shared Living)
In shared living, an individual lives with a trained Home Care Provider in the provider’s home. It is a family-style setting where the person is part of the household, not a guest. Each person takes part in choosing their Home Care Provider, so the match feels right for everyone.
Home Care Providers help with:
- Daily living skills like cooking, cleaning and personal care
- Medications, health appointments and healthy meals that meet dietary needs
- Getting out in the community, and staying connected with family and friends
- Working toward the goals in the person’s service agreement
Our team visits each home regularly.
Community Participation Services (CPS)
CPS helps people spend their days doing things that matter to them. That can include volunteering, recreation, learning new skills, exploring work and making friends. Each schedule is built around the person’s interests and goals, with the level of staff support they need.
Respite
Caring for someone full time is hard work. Respite gives families and Home Care Providers a short break, while the person gets safe, caring support from a trained respite provider.
Behavior Support
Some people need extra support to manage stress or difficult situations. Our behavior specialist works with the person and their team to create a behavior support plan, and trains caregivers to follow it. Our staff are trained in de-escalation, CPR and first aid.
Nursing Support
Our nurses oversee medication and health support in every home, in line with He-M 1201 State Regulations.
- Train and authorize Home Care Providers to give medications safely
- Visit each home regularly to review medications and medication records
- Review each person’s health needs and support the team with health questions
This support is part of our residential services.
How to get started
Services are arranged through your Area Agency
If you or your family member gets developmental disability services in New Hampshire:
- Ask your service coordinator about Home For Butterflies, or contact us directly.
- We meet with you and your team to learn about your needs and goals.
- We find a good match with one of our Home Care Providers or programs.
- Services start once your Area Agency approves them.
